Dihydrotestosterone — DHT — is the single most researched molecule in hair loss biology. In men with a genetic predisposition to androgenetic alopecia, DHT systematically shrinks follicles over years, producing the characteristic recession and thinning of male pattern baldness. Understanding this mechanism is the first step to interrupting it.
DHT is produced when testosterone is converted by an enzyme called 5-alpha reductase (5-AR), present in high concentrations in scalp tissue. In genetically sensitive follicles, DHT binds to androgen receptors in the dermal papilla — triggering a cascade of events that progressively shortens the anagen phase, reduces follicle size, and produces thinner, shorter, lighter hairs with each cycle.
Eventually, the follicle miniaturizes to the point of producing only vellus (fine, colorless) hairs — or stops producing hairs at all. This process can begin as early as the late teens and accelerates throughout a man's 20s and 30s.
The key variable is not testosterone levels — it's the density and sensitivity of androgen receptors in scalp follicles, determined largely by genetics. Men with high receptor density in the frontal and vertex regions are disproportionately affected, which is why pattern baldness follows predictable geometric patterns.
Rosemary extract (Rosmarinus officinalis) has been shown in multiple studies to inhibit 5-alpha reductase activity, reducing the conversion of testosterone to DHT at the scalp level. A 2024 clinical trial found rosemary oil statistically equivalent to 2% minoxidil in hair count improvement after 6 months — while also demonstrating 5-AR inhibition that minoxidil does not.
Saw palmetto (Serenoa repens) is another well-researched natural 5-AR inhibitor, with a 2021 meta-analysis concluding significant improvements in hair density in men with early-stage androgenetic alopecia. Pumpkin seed oil has similarly shown 5-AR inhibitory properties in randomized trials.
